Understanding Treatment-Resistant Depression (TRD)

Treatment-resistant depression is generally diagnosed when a person does not experience meaningful improvement after trying two or more antidepressant medications at adequate doses and duration.

TRD can involve persistent symptoms such as:

  • Ongoing low mood or sadness
  • Loss of interest or motivation
  • Changes in sleep or appetite
  • Difficulty concentrating
  • Feelings of hopelessness

 

When standard treatments are not effective, further evaluation may help identify additional treatment options.

Brain-Based Treatment Options for TRD

Depression is often associated with changes in brain activity and communication patterns. rTMS therapy targets specific brain regions involved in mood regulation through controlled magnetic stimulation.

In addition to FDA-approved rTMS, our clinic offers MeRT® (Magnetic e-Resonance Therapy)—an individualized, EEG-guided approach developed at our Newport Beach center. When used, MeRT incorporates diagnostic data, such as quantitative EEG findings, to inform treatment planning for select patients.

Determining the appropriate approach depends on clinical evaluation, diagnostic findings, and individual patient factors.

Evaluation and Treatment Planning

Care typically begins with a comprehensive evaluation, which may include:

  • Review of medical and treatment history

  • Quantitative EEG (qEEG) and EKG testing

  • A clinical consultation to discuss findings and potential options

If treatment is recommended, a personalized plan is developed based on clinical assessment and diagnostic data.

Treatment sessions are non-invasive, medication-free, and typically last about 30–45 minutes. Treatment schedules and duration vary depending on the individual and the approach used.

As with any medical treatment, response varies, and outcomes cannot be guaranteed.

Insurance & Coverage

Brain Treatment Center Newport Beach is in-network with TRICARE for FDA-approved rTMS therapy. Coverage eligibility depends on diagnosis, medical necessity, and plan details.

MeRT is considered an individualized, EEG-guided approach and is not covered by TRICARE. For other insurance plans, we are out of network. Our team can provide documentation to help patients inquire about possible reimbursement for eligible services.

We do not accept Medicare or Medicaid at this time.

Absolute Contraindications

Absolute contraindications for cortical MeRT treatment: Pacemaker, Defibrillator, Vagal Nerve Stimulator, VP Shunt/ Magnetic intracranial shunts, Deep Brain Stimulator, Epidural Cortical stimulator, Steel shunts/stents, Cranial metal fragments (i.e. shrapnel, excluding titanium), Cochlear implant, Aneurysm clips, coils, pipelines flow diversion, Pregnant or breastfeeding, Primary brain cancer / metastatic lesions in brain (unless palliative care), Magnetic dental implants, Implanted cardio-verter defibrillators (ICD), Ocular implants.

Relative Contraindications

Relative contraindications require closer protocol attention and may or may not disqualify someone from receiving cortical MeRT treatment, depending on the doctor’s discretion and the person’s individual condition. These include: History of Seizure or seizure disorder, Titanium shunts/stents, Spinal Cord Stimulator, Hearing aids, Ferrous cortical implants, Magnetic ink tattoo, Bipolar Disorder Type I/II, Baha Implant.
